This notice describes the current website enquiry form, not a policy for every future C & C service.
Who provides this website
This website and the enquiry form are provided by C & C Performance and Wellness Consulting LLC, a limited liability company registered in Arizona. Questions about this notice or your enquiry go to Cristo at cristo@ccperformancewellness.com.
What you provide
The form saves your name, email, message, enquiry category and preferred follow-up method. A phone number is optional for email follow-up and required if you select phone or text. Please avoid including medical records or sensitive clinical details; C & C is not a clinical care provider.
Page and referral information
The form also records the page address, referring page and any campaign labels present in the link you used, such as utm_source and utm_campaign. These are stored separately from your contact fields to provide context about how an enquiry reached the site. Form-start and form-submit events do not include your name, email, phone number or message.
What happens when you submit
Your enquiry is saved in the website’s private enquiry records for C & C to review. Other website visitors cannot read those records. Saving happens before the email draft opens, so closing or not sending the draft does not remove the saved enquiry.
The form prepares an email addressed to Cristo at cristo@ccperformancewellness.com. It is not sent automatically: you choose whether to send it from your email app. Your email service handles the draft and any email you choose to send under its own policies.
Correction, deletion and retention questions
To ask what information is held about your enquiry, request a correction or deletion, or ask how long it will be retained, contact cristo@ccperformancewellness.com. Include enough context to identify the enquiry without sending unnecessary sensitive information.
This website does not promise an automatic deletion schedule. Ask Cristo about the handling and retention of your specific enquiry before sharing information if that is important to you.
